This privacy policy describes the information that is collected, how it is used, and shared. PPS collects personal information for the purpose of creating a client file and to learn more about client needs through information provided on client questionnaires.

All information provided to PPS is kept confidential. The client may access their information through the client portal at any time. This portal is password protected. The client may make corrections to their personal information. This information is not shared with any other party unless requested by the client. In some circumstances, I may be obligated to share personal information if there is an obvious risk of harm to you or someone else, or in case of a medical emergency requiring emergency services.

Clients have the right to access their information, amend incorrect information, and withdraw consent to share information. PPS is not a healthcare provider, and therefore does not collect healthcare information. However, some aspects of the client’s perinatal care may be included in questionnaires for context and to ensure optimal delivery of support services.
